When a patient is recovering from an injury or illness, a physical therapist can develop a plan of care to help a patient restore daily functional abilities as well as regain or strengthen the use of their muscles and joints. With home health physical therapy, a licensed therapist comes to your home to work with you, following a specific treatment plan. Through a combination of hands-on therapy and guided exercises, physical therapists help patients transition to independence in daily living activities. Our Physical Therapy services are inclusive of, but not limited to the following:

• Restoration of range of motion, strength and balance
• Ambulation and mobility training
• Joint replacement recovery (knee, hip, etc.)
• Hands-on therapy with appropriate technology
• Training for transfer to bathtub, chair, bed, etc.
• Fall prevention

• Prosthetic training
• Home safety evaluations to identify safety risks
• Education for patients and families

For homebound patients, a home health Occupational Therapist can help a patient regain the ability to perform active daily living functions after an injury or illness. The Occupational Therapist brings these services directly to patients in their home environments to help improve daily function and gain independence. Occupational therapy may be provided as a home health service in conjunction with other skilled disciplines like physical therapy, skilled nursing or speech therapy. Our Occupational Therapy services are inclusive of, but not limited to the following:

• Strength, range of motion, and balance training for performing day-to-day tasks
• Personal-care training
• Therapy to address joint pain, tightness or stiffness in the arms, hands, or legs
• Wheelchair assessment and positioning training
• Home modifications
• Home safety evaluations to identify safety risks
• Education and training for family members

Our Certified Home Health Aide provides services under the supervision of our nursing and therapy team. Our Home Health Aide provides hands-on care to help patients with personal care and activities of daily living. Our home health aides may provide the following services:

• Activities of daily living including bathing, dressing, eating, grooming, moving, and toileting
• Check vital signs such as temperature, blood pressure, respiration and pulse
• Monitor the patient’s physical and mental condition
• Medication reminders

Communication is important for maintaining a good quality of life. When an injury or illness affects the ability to speak clearly and effectively, a speech therapist can help a patient regain the ability to communicate clearly. Our home health speech therapists evaluate, treat, train and educate patients with swallowing and communication disorders in the comfort of the patient’s home. Our Speech Therapy services are inclusive of, but not limited to the following:

• Breathing techniques
• Oral speech, strength, and coordination
• Swallow safety and techniques
• Word recall and use of language
• Adaptive communication equipment

Our Medical Social Workers help patients overcome some of the things that can get in the way of recovering from an illness or injury. Our Medical Social Workers perform home assessments to identify social, emotional, and financial concerns of the family & patient. Our Medical Social services are inclusive of, but not limited to the following:

• Assistance navigating community resources
• Family Counseling
• Health insurance coverage education
• Alternative living arrangements
• Assist with financial concerns
• Long-term care planning
• Provide information and assist with Advance Directives
• Case management
• Bereavement support
