With home health physical therapy, a licensed therapist comes to your home to work with you, following a specific treatment plan. Through a combination of hands-on therapy and guided exercises, physical therapists help patients transition to independence in daily living activities. Our Physical Therapy services are inclusive of, but not limited to the following:

  • Restoration of range of motion, strength and balance

For homebound patients, a home health Occupational Therapist can help a patient regain the ability to perform active daily living functions after an injury or illness. The Occupational Therapist brings these services directly to patients in their home environments to help improve daily function and gain independence. Occupational therapy may be provided as a home health service in conjunction with other skilled disciplines like physical therapy, skilled nursing or speech therapy. Our Occupational Therapy services are inclusive of, but not limited to the following:

  • Strength, range of motion, and balance training for performing day-to-day tasks

Home Health Aides

Our Certified Home Health Aide provides services under the supervision of our nursing and therapy team. Our Home Health Aide provides hands-on care to help patients with personal care and activities of daily living. Our home health aides may provide the following services:

  • Activities of daily living including bathing, dressing, eating, grooming, moving, and toileting
  • Check vital signs such as temperature, blood pressure, respiration and pulse
  • Monitor the patient’s physical and mental condition
  • Medication reminders

Speech Therapy

Communication is important for maintaining a good quality of life. When an injury or illness affects the ability to speak clearly and effectively, a speech therapist can help a patient regain the ability to communicate clearly. Our home health speech therapists evaluate, treat, train and educate patients with swallowing and communication disorders in the comfort of the patient’s home.
